When about to do an externship, please notify the Education & Residency committee once this is arranged. Please fill in the first half of the form below and send it to the college administration. The E&RC may come back to you for discussing further details.
After the externship, please fill in the second part and send it to the college administration. Try to complete both parts in one form for easier handling!
Please provide a certificate regarding the externship from the institution where it was done. If in doubt, contact the E&RC via the college administration.
